NJ MVC Forms & Downloads
Download the official New Jersey MVC forms you’ll need before starting your driving lessons or applying for your permit.
Divine Driving School provides these documents for your convenience. Please print, complete, and bring the required forms to your first lesson or permit appointment.
First time License, customers must walk-in to process their driving License. No appointment is available on the NJMVC website.
Most NJMVC report time for walk-in Customers is 8am. Newark NJMVC Customers should report and line up by 6:30am-7am to be given a pass to be served.
Essential Forms for New Drivers (Teens & Adults)
1. SPECIAL LEARNER’S PERMIT FORM (Form BA-412D)
Required for teens and adults applying for an examination permit. Must be completed prior to testing.

7. NJ MVC Parental Consent Form for Teens (Form STS-1)
Required for 16-year-old applicants entering the NJ Early Bird teen program (with a driving school).
8. CERTIFICATION OF SUPERVISED 50 HOURS LAW (FORM BA-CSD)
It must be completed and signed by a parent, guardian, or supervising driver and presented at the time the permit holder applies for a probationary license.
9. GDL (Graduated Driver License Restrictions)
Explains rules for 16-year-olds, 17-year-olds, and probationary drivers in New Jersey. A helpful reference for parents.

Before your first lesson, please:
- Download and complete all required forms
- Bring your valid permit, or the documents required to apply for one
- If you’re 16, confirm your MVC appointment with the driving school
- Bring any required vision or medical forms if applicable
