Divine Driving School provides these documents for your convenience. Please print, complete, and bring the required forms to your first lesson or permit appointment.
Essential Forms for New Drivers (Teens & Adults)
1. NJ MVC 6-Point ID Verification Requirements
This checklist explains the identification documents you must bring to the MVC when applying for a permit. You may also visit NJMVC website to access Required Documents: “6 points of ID”.
2. NJ MVC CERTIFICATION OF SUPERVISED DRIVING (FORM BA-CSD)
It must be completed and signed by a parent, guardian, or supervising driver and presented at the time the permit holder applies for a probationary license.
3. NJ MVC Driver Examination Permit Application (Form BA-412D)
Required for teens and adults applying for an examination permit. Must be completed prior to testing.
4. NJ MVC Parental Consent Form for Teens (Form STS-1)
Required for 16-year-old applicants entering the NJ Early Bird teen program (with a driving school).

Before your first lesson, please:
- Download and complete all required forms
- Bring your valid permit, or the documents required to apply for one
- If you’re 16, confirm your MVC appointment with the driving school
- Bring any required vision or medical forms if applicable
